Policies and General Information


  1. Some jobs could take more than 3 business days. Please call for information.
  2. ADDITIONAL CHARGES FOR:

  3. FREE HARD COAT for all Hi Index, Polycarbonate and Trivex lenses.
  4. RUSH ORDERS. A $15.00 service charge will be added for same day orders.
  5. PATIENT'S OWN FRAME AND LENSES. We do not assume responsibility for P.O.F.'s or lenses.
  6. NEW FRAME POLICY. On occasion a new frame may be defective and break during lens fabrication. If a frame breaks at a solder point, barrel or any stress point, we are not responsible and frame should be returned to the manufacturer for credit. Frames that are not suitable for Rx lenses are to be used at Dr.'s own risk.
  7. DOCTOR'S REDO. 50% Credit on lenses only for Dr.'s Redo with lenses and invoice returned in 45 days. One redo per job is allowed. Stock lenses are not covered.
  8. LAB ERROR. Lab must be notified of redo within 30 days after job has been received and defective lenses returned for credit with original invoice.
  9. OPTICIAN ERROR. We are not responsible for any errors due to work done on lenses outside our lab.
  10. NON ADAPT. 60 Days Non Adapt Warranty for Progressive Lenses. Lenses can be exchanged for a pair of FT28 or SV in CR39 Clear.
  11. INSURANCE. Add $10 to the cost of the job to get 100% credit in case of Dr.'s Redo with lenses and invoice returned in 45 days. One redo per job is allowed. In case of Non Adapt lenses should be returned within 60 days and can be exchanged for a pair of FT28 or SV lens of the same value or less.
  12. COATINGS. All coatings are covered for up to 1 year due to normal use. Zeiss Carat Advantage is covered for 2 years.
  13. DRILL MOUNTS. Polycarbonate is covered for 90 days against cracking & breaking.
  14. TRIVEX comes with a Lifetime Guarantee against cracking & breaking.
  15. CANCELLATION. Lenses not yet processed may be canceled at no charge. To cancel a job in process please call to verify cancellation fee.
  16. UNPAID ACCOUNTS Are subject to suspension of service.
  17. PRICES ARE SUBJECT TO CHANGE WITHOUT NOTICE.
